Scheduling a Meet and Greet is a great way for prospective parents and new patients to ask questions, see how we work, and meet one of our providers. We want to help you make the best decision for your family.

To do this successfully, we must be able to review all of your medical records from all providers that you have seen. It is our policy to receive your previous medical records before we can assume care and schedule an appointment.
Once the records are received, we will review them to determine if they are complete and sufficient for us to begin caring for your children. Unfortunately, we cannot treat or provide any care for your children until that time.
If you have not already done so, please sign a release of records form and provide it to your children's previous medical provider, we are happy to fax it for you.
Bringing a newborn to a pediatric practice can be a prudent decision. Pediatricians specialize in the knowledge of newborns and their rapid growth and development. A newborn should have regular checkups to monitor their growth and development, identify potential health issues early on, receive necessary vaccinations, and get guidance on proper infant care.

When transferring a child to our pediatric practice, we must receive all requested information before we can schedule an appointment for your child. You will need to download the Welcome Packet and Transfer In form for medical records. If you are unable to download and print forms you may come into our office to pick them up.
We will need:
A copy of your child's insurance card, (please make sure that we are participating with your particular plan)
Welcome packet completed
Medical records release so that we may request records from your previous provider. We must have previous records before we can schedule an appointment for your child.
